  • Where do I get an application form?
    You can get an application form from your PAH agent. You may contact us via e-mail or call us to find out further application information.
  • Can I send my application form by fax?
    You may not send your application form by fax. For group and individual insurance follow the following process: (1) Fill in the Ping An Health application form (2) Submit the original application form to an agent via post.

  • Where do I get a Declaration of Health form?
    Contact your Ping An Health agent for a Declaration of Health form.
  • If I am not familiar with medical terms, how can I fill in my Declaration of Health form?
    The main purpose of the "Declaration of Health" form is simply to explain your medical history. You need to fill in your basic physical information such as your current height and weight and then any other conditions that are or have been dangerous to your health, such as smoking, drinking and substance abuse; disabilities or imparities; surgery; nervous or psychological illnesses; accidents, any illnesses or diseases that have been dangerous to your health. You should also fill in your family medical history.
  • What documents should I attach to my Declaration of Health form?
    Attached to your Declaration of Health form you need to provide the following documents:
    1. Medical history documents issued by the hospital ( Inpatient and outpatient records: certificates of admission and discharge, diagnosis, discharge summary,outpatient medical records, etc.)
    2. The results of your latest medical examination.
  • How do I find out the requirements for policy administration?
    (a) Group customers: The policyholder should tell you the requirements for policy administration and authorize your documents. Either the policyholder will submit your documents for you, or you can send your authorized application form to Ping An Health with a copy to the policy holder.
    (b) Individual customers: You may submit your application to a PAH agent at your relevant Ping An branch.
  • What do I do if my insurance card gets lost or damaged?
    Call us directly on 4008833663 and we will arrange a new card for you.
  • Should I inform the insurance company if my residence changes?
    You should inform the your insurance agent so that your personal information can be updated.
  • What do I do if I want to renew my policy?
    (a) Individual customers: Our individual products are valid for a one-year insurance period and are renewable. If you choose automatic policy renewal, Ping An Health will issue a letter of renewal one month before your policy expires to confirm the premiums and conditions of this renewed policy. However, if you do not choose automatic renewal, you will have to re-purchase insurance.
    (b) Group Customers: Policy renewal does not apply to group customers, insurance will have to be re-purchased and new application forms should be filled out.
  • Which documents do I need to bring to a medical checkup at a hospital or check-up centre?
    You need to bring your passport to the hospital or designated check-up center on the day of your check-up.
